Ellen,

Sever's always responds to conservative care usually with an accommodative (soft) orthotic. I would recommend several options to aid in the diagnosis.

1. MRI
2. Blood tests including Rheumatoid factor, HLA B-27, ANA, Sed rate, c-reactive protein, cbc with diff, thyroid panel, comprehensive metabolic, and vitamin B12 level

It's likely one of the above will solve the diagnosis. I have seen cases of metabolic bone disease present this way. I hope this helps.
