Such a callous as you describe can occur if there is a skin fold on the plantar surface of the foot, perhaps occasioned by a loss of subcutaneous padding, allowing for there to be skin which is not taught and thus subject to irritation on weight-bearing. These are not generally the acutely painful type of callouses commonly seen under the metatarsal heads. If the aforementioned type is the case, it may be that something very simple, such as a strip of moleskin, will allow for enough slippage over this area, such that the callous might not occur or even go away. But without a hands-on evaluation, my comments are based entirely on supposition. See a podiatrist for a definitive evaluation and recommendations.
