I certainly don't know all there is to know regaring the specifics of your situation, but I would judge that many of the problems which you have indicated were the results of the surgical procedure that you underwent, are not, in fact, caused by that procedure.

In close to 3 decades of practice, I have never seen an arch "collapse" and I certainly can't see any likely connection between such an event, if, in fact, it did occur and any procedure which might be done to straighten toes.

From your description, I would not be terribly surprise if you did not have an excessive high-arched foot, know medically as pes cavus. This frequently results in dorsally contracted toes and, in addition is, in general, one of the more difficult foot deformities with which to deal.

It would also seem to me that you are discounting the effect of excessive weight, especially as an exacerbator of problems of the knees. If you are serious about your concern, I would suggest that you lose the weight and see what improvement you might obtain. Even if it's none, it's still a win-win situation.

I would doubt that irreversible damage has been done in regard to this situation by ANYONE other than yourself.
